If paying by check send completed form with your check to the address provided.

If paying by credit card proceed to the Paypal buttons. Be sure to choose the selection that matches your printed Registration Form and follow the PayPal instructions to complete your payment.

IMPORTANT: Print a copy of the PayPal receipt and send it with the Registration Form that you mail in.

There will also be on-site registration. Keep in mind there are limited sessions, so you should register ahead of time.

* You must validate your student status. "I am a active student in a degree seeking program."

Contact hours provided by:
National Association of Alcoholism and Drug Abuse Counselors (NAADAC) and Substance Abuse Certification Alliance of Virginia (SACAVA)
There are 4 sessions with 3 contact hours offered for each session. Lunchbox sessions will be offered with 1 contact hour awarded for each session.

Lunch Box Meals selections will be available Monday and Tuesday. Cost for Lunch Box meals is $15.00 and must be paid for with your reservation. There will be no ham, shellfish or peanut products served. You must pre-order.

Award’s Luncheon (Tuesday, July 17)
If you wish to attend, you must register and pay for this lunch with your registration. We will not be selling tickets to attend the luncheon at the conference. Limited to 150 tickets. Deadline to register for the Awards Luncheon is June 17, 2012. Cost $15.00.

Cancellation Fee
Notice of registration cancellation must be made in writing to the Registrar of VSIAS at VaSummerInstitute@verizon.net . If you cancel up until July 1, 2012, then there will be a $50 fee for canceling. If you cancel after July 1, 2012, then there will be a $75 fee for canceling. Reimbursement will be by check if there is a cancellation.

Credit for Attendance
VSIAS is an education provider for a number of credentialing and academic organizations. VSIAS exercises its ethical responsibility to ensure that contact hours are earned fairly and honestly by awarding contact hours for session attendance only if an individual attends a minimum of 80 percent of that session. VSIAS Faculty cannot make exceptions to this policy and will take required professional action in cases of misrepresentation.

Hotel Accomodations:
The above rates are State/Federal Per Diem. All rates are subject to state and local taxes, currently 10% sales tax (state and local sales taxes combined) and $2.00 per room per night Williamsburg occupancy tax. Taxes are subject to change.

In the event that the state government per diem rises prior to the arrival date, the guest room rate will be adjusted accordingly. In the event that the per diem rate drops prior to the arrival date, the guest room will remain $91.00.

You may make your hotel reservations directly with the Hospitality House (757-229-4020 or 1-800-932-9192). Reference the Virginia Summer Institute for Addiction Studies for the conference rate of $91.00 per night, plus taxes. All reservations must be made by June 14, 2012.

Check-in time is 3:00 PM and check-out time is 11:00 AM. Please do not arrive to check-in before 3:00 PM, as the hotel does not have enough space to store everyone’s suitcases and materials.
